**Please note that six references are required for all instructional positions. Print six copies of this form, and distribute to your references. Have those individuals return the completed forms directly to the Human Resources Department of CSCS.

Mission & Doctrinal Statements/Conduct Standards A signed copy of this form is required with all applications.

For instructional and administrator positions, please submit a copy of your college transcripts and your ACSI Certificate if you have one.
